提供测试产品是否符合上述消费品安全规则的日期及地点:

提供测试的地点及认证所依据的测试日期或测试报告。

认证证书所依赖的第三方实验室的识别:

一般来说，对于GCC，这个部分应该标记为“N/A”，因为第三方实验室测试不是非儿童产品的必要条件。(这只是对儿童产品的要求，必须包含在CPC中。)但是，如果认证机构自愿使用第三方实验室的测试结果作为签发GCC的依据，法律要求认证机构必须提供第三方实验室的名称、完整的邮寄地址和电话号码。
